How we handle your information

The Mental Wellness Tribe is a community initiative operated by The Trustee for Wealth Creators Network Trust ABN 18 759 783 233, with Wealth Creators Australia Pty Ltd as trustee.

Information we collect

We may collect your name, email address, phone number, location, age group, referral source, wellness interests, goals and written responses that you choose to provide. Some responses may be health-related or otherwise sensitive.

Why we collect it

We use this information to respond to your request, understand what support or education may be relevant, communicate possible starting points and manage our relationship with you. Completing the form does not create a medical, psychological, therapeutic or practitioner-client relationship.

Who may review it

Authorised members of our support team, including Corey Sievers and Linda Barker, may review your submission. If another person referred you, we may discuss relevant information with that person so they can participate in responding to your request. We limit this sharing to what is reasonably relevant to that purpose.

Email delivery and service providers

Form submissions are transmitted through FormSubmit, a form-email delivery provider, to our Google Workspace or Gmail account. FormSubmit may process or store information outside Australia and states that submissions may be retained in its submission archive for up to 30 days. Google may also process or store information outside Australia. We use the information for the purposes described here and do not sell it.

Marketing

Marketing permission is separate and optional. If you opt in, we may send wellbeing information, resources and updates. You may unsubscribe at any time. We do not use sensitive wellness information for unrelated marketing.

Security and retention

We take reasonable steps to protect personal information and restrict access to people who need it to respond. No internet transmission or email system is completely secure. Apart from FormSubmit’s service retention described above, we retain information only as reasonably required for communication, administration and applicable legal obligations.

Access, correction and questions

You may ask what information we hold about you, request a correction or raise a privacy concern by emailing hello@thementalwellnesstribe.com.
